What You’ll Do
- Provide hospice nursing visits in patient homes and facility settings
- Deliver pain and symptom management, clinical assessments, and patient education
- Support patients and families through end-of-life transitions with confidence and compassion
- Collaborate closely with physicians, social workers, chaplains, and care team members
- Communicate clearly with families, facility staff, and referral partners
- Complete accurate, timely documentation in the EMR
Who We’re Looking For
- Active Minnesota RN license
- Experience in hospice, home care, palliative care, oncology, or end-of-life nursing
- Comfortable working independently in the field
- Strong communication skills and emotional intelligence
- Reliable transportation and valid driver’s license
- A nurse who wants to help build something meaningful
