Your Key Responsibilities:
- Conduct nursing assessments and monitor patient conditions
- Deliver medications, treatments, and skilled interventions
- Educate patients and families on safety and home care routines
- Assist with daily living needs while promoting independence
- Document care accurately and communicate updates to the RN and care team
Qualifications:
- Active LPN license in Minnesota
- 1 year of clinical or home care experience
- Strong communication, organization, and problem-solving skills
- Ability to work independently in the field
- Reliable transportation for patient visits
