Responsibilities

• Perform initial assessments and ongoing evaluations to determine physical therapy needs
• Develop individualized treatment plans and implement therapeutic interventions to help patients reach functional goals
• Provide outpatient therapy with occasional LTC/Skilled Swing coverage, documenting progress and revising plans as needed
• Educate patients and families on home exercise programs, safety, and activity modification
• Collaborate with other health professionals and ancillary staff to coordinate care and optimize outcomes
• Maintain accurate documentation and communication to support patient progress and discharge planning
  
  

Requirements

• Active Physical Therapist license in Montana or eligibility for Montana licensure
• Ability to practice in an outpatient setting with a diverse patient population (approximately 95% outpatient, ~5% LTC/Skilled Swing)
• Strong communication and teamwork skills to collaborate with the health care team
• Availability to commit to a 13-week travel assignment with 40 hours per week
• Demonstrated ability to assess, plan, and document patient care and progress
