Responsibilities

• Provide direct nursing care to residents under the supervision of an RN, including vital signs, assessments, and routine procedures
• Administer prescribed medications and document administration accurately in the charting system
• Monitor patient status, respond promptly to changes, and escalate concerns per policy
• Assist residents with activities of daily living and mobility; implement dementia care plans as required
• Collaborate with the interdisciplinary team and document communications; support families as appropriate
• Float to other units as needed to meet patient needs and census demands
  
  

Requirements

• Active LPN license in good standing
• BLS certification (American Heart Association)
• Minimum 3 years of long-term care experience; prior travel experience preferred
• Comfort working with residents who have dementia
• Ability to float to other units as needed
• Every-other-weekend availability required
• Availability for 3x12-hour night shifts
