Responsibilities

• Conduct comprehensive occupational therapy assessments for residents to identify limitations in activities of daily living, mobility, and cognitive function
• Develop and implement individualized treatment plans to improve independence in daily activities, upper-extremity function, coordination, and dexterity
• Provide therapeutic interventions to restore or compensate for impairments, including activities that promote safety, independence, and quality of life
• Collaborate with an interdisciplinary team (nursing, rehabilitation therapists, social work) to align goals and coordinate care
• Document evaluations, progress notes, goals, discharge planning, and outcome measures in accordance with regulatory and facility guidelines
• Educate residents, families, and care staff on adaptive equipment, activity modifications, and home/activity modification strategies
  
  

Requirements

• Active Occupational Therapist license in Delaware or eligibility per state requirements
• Ability to work in a long-term care setting with residents
• Strong documentation, communication, and interpersonal skills
• Willingness to participate in a 13-week travel assignment and meet guaranteed hours
• Solid clinical assessment and treatment planning capabilities
