Responsibilities

• Conduct initial and ongoing PT evaluations for residents, focusing on mobility, strength, balance, and functional independence
• Develop and implement individualized treatment plans to improve activities of daily living and gait, using evidence-based physical therapy interventions
• Administer therapeutic exercises, manual therapy, modalities, and assistive devices as appropriate to patient goals
• Monitor progress, document outcomes, and adjust treatment plans to align with resident goals and care team recommendations
• Collaborate with interdisciplinary team members (nursing, OT, speech therapy, social work) to coordinate care and facilitate smooth transitions in care plans
• Ensure documentation compliance, maintain accurate records, and adhere to regulatory and facility policies
  
  

Requirements

• Active Physical Therapist license (state-specific) or eligible to practice in Wyoming
• Ability to work effectively in a long-term care setting and with an older adult population
• Strong communication and documentation skills; collaborative teamwork mindset
• Willingness to commit to a 13-week travel assignment and Day shift schedule
