Responsibilities
- Perform comprehensive PT evaluations on residents, interpret functional impairments, and document findings to guide evidence-based treatment plans within regulatory standards.
- Design and implement individualized therapy interventions including gait training, therapeutic exercises, balance and proprioception work, ROM and strengthening programs, transfer and mobility training, and use of assistive devices as appropriate.
- Monitor patient progress, adjust plans of care as needed, and communicate results to the interdisciplinary team; maintain accurate documentation in the electronic health record per facility policy.
- Educate residents and family/caregivers on home exercise programs, safety strategies, fall prevention, and energy-conserving techniques to optimize independence.
- Collaborate with physicians, nursing staff, occupational therapy, speech-language pathology, and therapy aides to coordinate care and ensure patient safety, infection control, and compliance with accreditation and regulatory requirements.

Requirements
- Active Physical Therapist license to practice in the United States; state-specific licensure required for assignment.
- Doctor of Physical Therapy (DPT) degree or equivalent per licensure requirements.
- Ability to work effectively in a long-term care/nursing home setting with geriatric patients and complex care needs.
- Strong communication and interpersonal skills; ability to collaborate with an interdisciplinary team and document clearly.
- Willingness to travel for a 13-week assignment and to work a day shift with guaranteed hours.
