Responsibilities

• Provide direct patient care to residents under supervision of a licensed nurse in a long-term care setting
• Administer medications and document administration according to care plans and facility procedures
• Monitor vital signs, assess changes in resident condition, and report to the supervising nurse
• Assist residents with activities of daily living and mobility, promoting safety and dignity
• Perform basic wound care, dressing changes, and routine procedures per policy
• Document care provided in the electronic health record and communicate clearly with the care team
  
  

Requirements

• LPN licensure in Wisconsin or eligibility for licensure (per standard care practice in a LTC travel role)
• Ability to work evenings and nights in a rotating schedule
• Comfort and experience in long-term care settings; ability to follow care plans and document accurately
• Strong communication, critical thinking, and teamwork skills
• Ability to pass background check and health screenings required for clinical deployments
