Responsibilities

• Provide direct patient care to residents according to the plan of care under supervision
• Administer medications and document administration in the resident chart
• Monitor vital signs, assess changes, and report findings to the supervising clinician
• Assist residents with activities of daily living and mobility as needed
• Collaborate with the interdisciplinary team, including RNs, physicians, and therapy staff
• Maintain accurate documentation, uphold safety and infection control standards, and adhere to facility policies
  
  

Requirements

• Active Licensed Practical Nurse license in Vermont in good standing
• Graduate of an accredited practical nursing program
• 1 year of recent experience in a long-term care or similar setting preferred
• Ability to work a flexible travel assignment and meet standard credentialing requirements
• CPR/BLS certification preferred
• Strong communication and teamwork skills; ability to document clearly and accurately
