Responsibilities

• Provide licensed nursing care to long-term care residents per the plan of care
• Administer prescribed medications and treatments following orders and facility policy
• Monitor residents’ conditions, document findings, and report changes to the RN and medical team
• Assist with activities of daily living and restorative care, including mobility, hygiene, and nutrition
• Maintain a safe, clean, and compliant care environment; uphold infection control practices
• Communicate effectively with residents, families, and interdisciplinary team members to support quality outcomes
  
  

Requirements

• Active Licensed Practical Nurse license in Vermont (or eligible to practice in-state)
• Experience in long-term care or skilled nursing settings preferred
• Ability to work day and/or evening shifts as required
• Strong documentation and communication skills
• Willingness to participate in a travel assignment and work in a skilled nursing care setting
