Responsibilities

• Provide direct nursing care to residents under supervision and in coordination with the care team
• Administer prescribed medications and document administration accurately
• Monitor vital signs, safety needs, and changes in residents’ conditions; report promptly
• Perform routine nursing procedures within scope of practice and facility protocols
• Assist residents with activities of daily living and mobility, supporting independence and dignity
• Document care in the electronic health record and communicate changes to the supervising RN or charge nurse
  
  

Requirements

• Active Licensed Practical Nurse license in New Hampshire
• Experience or demonstrated ability to provide care in a long-term care or skilled nursing setting
• Ability to work 12-hour night shifts in a fast-paced environment
