Responsibilities

• Provide direct nursing care to residents in a memory care unit, including routine assessments and documentation
• Administer medications and treatments per physician orders; monitor for adverse reactions and effectiveness
• Monitor residents’ conditions, respond to changes, and coordinate care with the interdisciplinary team
• Support activities of daily living, mobility, and comfort measures; assist with transfers and fall prevention
• Conduct basic wound care, monitor treatment plans, and update care plans as needed
• Ensure safety, privacy, and respect for residents; maintain accurate charts and compliance with regulatory requirements
  
  

Requirements

• Active LPN license in Vermont
• Diploma or certificate from an accredited practical nursing program
• Ability to work in a memory care setting and collaborate with a multidisciplinary team
• Experience in long-term care or memory care settings preferred (not required)
