The Case Manager’s primary function is to enhance the quality of client care, to maximize client satisfaction, and to promote client’s overall wellbeing while in the recovery process by providing support as needed across all facility locations. Works to facilitateclient care by assessing client needs, evaluating treatment options, coordinating care, and facilitating individual, family and group counseling. The overall goal is to improve clinical outcomes, increase client satisfaction, and promote cost-effectiveness.
Duties include, but are not limited to assessing, planning, and implementing actions required for the client’s recovery needs: facilitating individual and group counseling sessions and providing on-site and virtual crisis support.
Job Functions & Responsibilities
The essential functions include, but are not limited to the following:
- Provides support and clinical coverage when other clinical staff members are unavailable and as assigned by manager.
- Gathers data of clients’ history through personal interviews with client, family, and others; prepares case histories affecting client; provides clinical assessment and treatment planning for clients.
- Conducts intake assessments and documentation within 24 hours of admission (including but not limited to ASAM, BPS, Treatment Plans, Patient Assessments, etc.).
- Manages a client caseload as assigned- oversees completion of all case management needs (court/legal issues, FMLA/STD, appointments, aftercare, communal services, etc.).
- Conducts one weekly individual case management/counseling session and provides weekly family check ins.
- Facilitates Emergency Contact calls per Company’s Policies and Procedures and ensures consistent communication with clients’ support systems.
- Assists with completion of Utilization Review documentation one day prior to UR dates.
- Completes and updates treatment plans (weekly) for each client on caseload.
- Facilitates group counseling session as assigned.
- Ensures outcomes assessments and surveys are diagnostically appropriate and completed.
- Prepares and coordinates discharge and aftercare plans.
- Supports Counselor in obtaining clinical documentation and coordination of care.
- Collaborates with the Financial Department to ensure all required insurance payments are collected and disseminates pertinent client information that would prevent payment collections.
- Participates in assigned trainings and proactively seeks knowledge to build competency in specialty treatments provided by company.
- Maintains qualifications and provision of care in accordance with national acceptable standards of practices pertinent to roles based on job title.
- Maintains knowledge of current regulatory and accreditation standards, along with federal, state, and local codes pertinent to this role and addiction treatment.
- Ensures all regulatory standards, requirements, and guidelines, including licensing boards, accreditation authorities, and federal, state, and local regulations are adhered to.
- Collaborates with medical team and other clinical staff in the development and implementation of clients’ treatment and intervention programs that they can understand and comply with.
- Participates in crisis intervention and conflict resolution actions, including suicide assessments, partner abuse, child and elder abuse screening; takes appropriate measures to resolve crisis situations; implements mandatory reporting procedures in accordance with regulations (including HIPAA) and policies.
- Monitors clients’ conduct throughout the day.
- Ensures clients are attending appropriate sessions and are on time.
- Collaborates with clinical team and attends and participates in assigned clinical team meetings.
- Conveys clients’ needs to medical, operational, and other clinical staff.
- Maintains a high level of flexibility with the daily flow of work tasks.
- Maintains proper chain of command.
- High School diploma or equivalent preferred
- 2 years’ experience in case management; preferred
- An active Substance Use Disorder (SUD) intern registration (e.g., RAC, RADT, SUDRC or equivalent registration), an active Substance Use Disorder (SUD) certification (e.g., SUDCC, CADC, CATC, or equivalent certification), or an active therapeutic license (e.g., LMFT, LPCC, LCSW, etc.) or registration (e.g., AMFT, APCC, ACSW, etc.)
- Effective October 1, 2024 Enrolled in an educational program toward certification within 6 months of placement in role
- First Aid/CPR/AED certified on first day of placement in role
- Effective September 1, 2023, a valid Motor Vehicle License
- Continue education requirements to meet yearly certification renewals
