This is a full-time, on-site role for a Case Manager located in Downey, CA. As a Case Manager, you will be responsible for assessing client needs, developing personalized action plans, coordinating care and resources, and advocating on behalf of clients to ensure they receive necessary services. You will regularly monitor client progress, maintain accurate case documentation, and collaborate with multiple stakeholders, including community organizations, healthcare providers, and internal teams, to achieve positive outcomes for your clients.
- Strong case management skills, including assessing needs, planning, and case coordination
- Excellent organizational, time management, and documentation skills
- Effective interpersonal, communication, and advocacy skills for collaborating with clients, families, and service providers
- Understanding of healthcare systems, community resources, and social services
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
- Experience working with diverse populations and individuals in need preferred
- Proficiency in computer applications and case management software
- Compassionate and solution-oriented approach to working with clients
