- Follow up on residents out in the hospital
- Create a protocol to follow up on hospitalizations for our residents in the facilities and skilled nursing facilities.
- Follow up on residents’ enrollments for ALW/ disenrollments
- Follow up on cal aim authorizations and ensuring medical health plans are correct
- Conduct comprehensive assessments of residents' needs, including medical, social, and housing needs.
- Develop individualized care plans in collaboration with residents, healthcare providers, and other stakeholders.
- Coordinate access to medical care, mental health services, substance abuse treatment, and other supportive services as needed.
- Advocate for residents' rights and access to resources, including housing, benefits, and community support services.
- Monitor residents' progress towards goals and adjust care plans as needed to promote positive outcomes.
- Provide crisis intervention and support to residents experiencing challenges or setbacks.
- Maintain accurate and up-to-date documentation of case management activities and resident progress.
- Previous experience in case management or care coordination, preferably in a healthcare or social services setting
- Compassionate and empathetic attitude towards individuals from diverse backgrounds, including the underserved population.
- Knowledge of community resources and social service programs available to individuals in need
- Strong interpersonal and communication skills, with the ability to build rapport and establish trust with diverse populations
- Excellent organizational and time management abilities, with a keen attention to detail
- Ability to work independently and as part of a multidisciplinary team in a fast-paced environment
- Proficiency in electronic medical records (EMR) and documentation software preferred
- Valid driver's license and reliable transportation required
