Title - RN Case Manager
Location - Houston, TX
Duration - 13 weeks
Shift - 8am to 4:30 pm.

Overview
Cambay Healthcare is seeking a dedicated and compassionate RN Case Manager to join our healthcare team. Responsibilities
- Assess patients' needs through comprehensive evaluations and develop personalised care plans.
- Coordinate multidisciplinary services to ensure seamless delivery of healthcare interventions.
- Monitor patient progress and adjust care plans accordingly, maintaining detailed documentation.
- Educate patients and their families about treatment options, medication management, and health maintenance strategies.
- Collaborate with physicians, specialists, and other healthcare professionals to optimise patient outcomes.
- Advocate for patients’ needs within the healthcare system, ensuring they receive appropriate resources and support.
- Maintain up-to-date knowledge of relevant healthcare regulations, policies, and best practices.
Qualifications
- Registered Nurse (RN) qualification with current registration recognised in the UK.
- Proven experience in patient care within a clinical setting.
- Strong knowledge of human anatomy and physiology.
- Excellent organisational skills with the ability to manage multiple cases efficiently.
- Compassionate approach with excellent communication skills for engaging with patients and multidisciplinary teams.
- Ability to work independently as well as part of a collaborative team environment.

Facility Requirements:
- Must have BLS certification must be attached to submission packet (Must be through the American Heart Association)
- Must have mininum 5 years recent RN Case Management experience, with experience in a LTAC or STACH setting, MCG, discharge planning across acute care settings, strong IDT collaboration
- Must have Texas RN or multi-state RN license (must be attached to resume)
- Shift is Monday - Friday 8a-430p
