What are the responsibilities and job description for the Hospice RN Case Manager position at Viking Hospice Care?
$75,000–$85,000/year | Full-Time | Houston, TX | Field-Based
Viking Hospice Care is a locally owned, Medicare-certified hospice agency serving Greater Houston. We are hiring our first RN Case Manager to lead patient care and help build our clinical program from the ground up.
As an early-stage agency, you will be instrumental in building the clinical infrastructure of Viking Hospice Care alongside our clinical and operational leadership. This role offers genuine ownership and direct impact that are rarely available at large corporate agencies.
What You’ll Do
- Manage a hospice patient caseload with visits in homes, ALFs, and SNFs across Harris, Fort Bend, Montgomery, and Brazoria Counties
- Conduct comprehensive clinical assessments, develop plans of care, and manage symptom control and end-of-life comfort care
- Facilitate IDT meetings and maintain documentation in compliance with CMS Conditions of Participation
- Conduct admission visits including full consent and plan of care initiation
- Complete all documentation in Hospice Tools on the day of service
- Perform death pronouncements and manage controlled substance disposal per DEA requirements
- Coordinate with the Medical Director, pharmacy, and DME partners on patient needs
Requirements
- Current, unrestricted Texas RN license in good standing
- Minimum 1 year of experience in a Medicare-certified hospice setting
- Experience with IDT meetings, care planning, and hospice admissions
- Comfortable working independently in the field
- Reliable vehicle and willingness to travel throughout Greater Houston
- Smartphone capable of running Hospice Tools (iOS or Android)
Preferred
- 2 years of hospice RN case management experience
- CHPN certification
- Bilingual in English and Spanish
- Familiarity with Houston’s hospitals, SNFs, and ALFs
- Prior experience at a startup or independent hospice agency
Compensation & Benefits
- $75,000–$85,000 per year, based on experience
- Health insurance, 401(k), and PTO
- Mileage reimbursed at the IRS standard rate
