Travel RN Case Manager
Location: Houston, Texas

Job Overview:
We are seeking an experienced RN Case Manager for a travel assignment at Kindred Hospital Houston Medical Center. This role focuses on coordinating patient care, optimizing resource utilization, and ensuring high-quality outcomes through effective case management and discharge planning.
Key Responsibilities
- Coordinate and manage patient care plans
- Perform comprehensive patient assessments
- Collaborate with interdisciplinary teams (IDT)
- Ensure compliance with care standards and external review agencies
- Manage utilization review and discharge planning
- Advocate for patient needs and continuity of care
- Support quality improvement and cost-effective care delivery
Requirements
- Minimum 5 years of recent RN Case Management experience
- Experience in LTAC or STACH settings
- Strong knowledge of MCG guidelines & discharge planning
- Excellent interdisciplinary team collaboration skills
- Active Texas RN License or Multi-State License (Required)
- BLS Certification (American Heart Association) (Required)
Additional Details
- Must provide all required documents at the time of submission
- Previous experience with Kindred facilities must be disclosed
- Availability for interview within 72 hours is preferred
Pay: $2,200.00 - $2,300.00 per week
