Bilingual Field Case Manager Compensation

• Compensation $34.13/hr

Bilingual Field Case Manager Position Highlights

• Fully remote role with approximately 75% local field travel to members’ homes, assisted living facilities, and nursing facilities
• Full-time | 40 hours per week
• Monday–Friday, 8:00 AM–5:00 PM EST
• Training conducted remotely via Microsoft Teams (4–6 weeks)
• Must reside within the assigned service area
• Bilingual Spanish/English required

Bilingual Field Case Manager Responsibilities

• Conduct comprehensive assessments through both in-home and telephonic visits
• Evaluate eligibility and determine appropriate supports based on member healthcare needs
• Develop, implement, and monitor individualized care plans
• Coordinate services with PCPs, specialists, and community-based providers
• Identify internal and external resources, including community supports, medication reviews, and condition management programs
• Manage prior authorizations, referrals, and benefit coordination
• Utilize case management guidelines, policies, and regulatory standards consistently
• Use motivational interviewing, negotiation, and advocacy to support positive member outcomes
• Provide coaching and education to promote healthy lifestyle choices and informed healthcare decision-making
• Maintain accurate and timely documentation, follow-up, and communication
• Travel locally to member residences and long-term care facilities within the assigned territory

Bilingual Field Case Manager Required Qualifications

• Bilingual Spanish/English (reading, writing, speaking) REQUIRED
• Bachelor’s degree required (Social Work or related human services field; RN candidates not considered for this role)
• Case management experience required
• Long Term Care (LTC) experience preferred
• Proficiency with Microsoft Office, including Excel
• Strong written, verbal, and interpersonal communication skills
• Ability to multitask, prioritize, and adapt effectively in a fast-paced environment
• Ability to work independently and manage a complex caseload
• Must reside within Miami-Dade County and be comfortable with frequent local travel
• High School Diploma or GED (verifiable) required

Bilingual Field Case Manager Preferred Experience

• Experience with Medicaid, LTC, or Managed Care programs
• Prior field-based or in-home case management
• Motivational interviewing skills
• Knowledge of community resources within Miami-Dade County
