Field Case Management Coordinator
Location: Field-Based, Illinois (Cook County & Surrounding Suburbs)
PayRate: $40.00/hr
Field & Telephonic Role
Field Case Management Coordinators to support elderly adults and individuals with disabilities approved for in-home or nursing facility care. This is a field-based, member-facing role requiring regular travel to members’ homes within Cook County and nearby suburbs. Between visits, work is performed remotely from home.
Candidates MUST be Illinois residents and MUST live in or near Cook County or surrounding suburbs. Each resume must clearly list the ZIP CODE the candidate is applying for.
Key Responsibilities
Member Evaluation
- Conduct comprehensive assessments using care management tools and available data.
- Review benefit plans and available internal/external programs to recommend solutions.
- Identify high-risk factors and care needs; escalate to clinical case management or crisis intervention when appropriate.
- Coordinate and implement care plan activities; monitor and document progress.
Care Quality & Clinical Collaboration
- Use a holistic approach to collaborate with case managers, supervisors, medical directors, and other programs.
- Present cases in multidisciplinary case conferences to achieve optimal outcomes.
- Identify and escalate quality-of-care concerns.
- Use negotiation and motivational interviewing strategies to promote member engagement and behavior change.
Member Empowerment & Support
- Provide coaching, education, and support to help members make informed healthcare decisions.
- Encourage members to actively collaborate with providers and follow care plans.
Monitoring, Documentation & Compliance
- Utilize case management and quality management processes consistent with regulatory, accreditation, and company standards.
Required Qualifications
- Bachelor’s degree in behavioral health or human services (Psychology, Social Work, Marriage & Family Therapy, Counseling, Nursing, etc.) OR non-licensed master’s-level clinician — required
- Illinois residency required
- High School Diploma or GED (verifiable) – required
- 2 years of experience in behavioral health, social services, or related field
- Case management experience – preferred
- Managed care experience – preferred
- Must be organized, efficient, and able to work independently
- Able/willing to travel frequently for in-home visits
