Purpose:
The Mental Health Case Manager for the homeless youth in our Transitional Living Program is tasked to
create an environment of trust and compassion that allows young adults to grow and develop. They will
fully assess the youth’s diverse needs, and provide psycho-social support, linkage to much needed mental
wellness resources and services, support with appropriate housing, and conduct aftercare services for our
Staying Strong Program. The Mental Health Case Manager will ensure that all individual plans and all
services are provided in accordance with the Youth Emerging Stronger mission statement, Value
Statement, Code of Conduct and YES Policies and Procedures. This position requires an individual who can
actively contribute to a positive, stable, responsive and responsible environment for change.
Responsibilities and Duties
Serve a caseload of approximately 15 homeless youth, providing active case management for all
assigned cases, including meeting weekly with each person individually
Manage caseload of assigned youth with an array of psychiatric, developmental and learning disabilities
Act as the lead for client’s housing-related case management, maintaining an active mental health plan
for each client on your caseload, based in creative solutions to mental well being
Maintain a current and thorough knowledge of community resources and utilize them to provide
comprehensive, wrap-around services to clients, navigating them through the best options for their
unique situations
Work collaboratively with youth to provide, refer, and/or link clients to support services needed. This
may include, but is not limited to mental health and co-occurring substance abuse rehabilitation,
physical health services, vocational, education and socializing, housing, money management, and
community integration.
Work with interdisciplinary team to identify barriers and resolutions to foster client independence
Develop effective, trusting relationships with clients and use a client-centered approach that includes
motivational interviewing, harm reduction, and trauma-informed care
Advocate for client’s needs in the TLP and when interfacing with other agencies or service providers
Create a supportive environment and ensure clients’ progress, utilizing a strengths-based approach
Maintain an attitude of professionalism, collegiality, and optimism with both staff and youth to
promote the welfare and well-being of the youth.
Work with all TLP case managers on identifying appropriate referrals for the “Be Well” Track
Facilitate wellness, enrichment, and life skills groups
Provide crisis intervention when needed
Maintain client confidentiality at all times while following agency regulations
Create and maintain accurate documentation of client information using Electronic Health Record/Exym
(referrals, progress notes and services)
Participate in client case conferences and individual supervision to provide information and make
recommendations for treatment, discharge planning, and client management.
Develop and implement realistic and relevant treatment goals
Formulate and implement a plan of treatment and rehabilitation which promotes client participation in
the development and maintenance of client’s individualized goals.
Work in collaboration with other agency and program staff, in order to coordinate services
Collaborate with Staying Strong Coordinator to locate appropriate housing solutions
Communicate effectively and in a timely manner with management, peers, and clients
Participate in evaluation and trainings in order to better respond to client/community needs
Must be willing to participate in YES’ rotating On Call schedule
Other duties as assigned
BASIC QUALIFICATIONS:
Ability to exercise good clinical judgment and act calmly in emergency situations.
Bilingual in Spanish preferred.
Experience and ability to work in a fast-paced environment with good time management skills.
Self-starter, independent worker with the ability to adapt to change in a fast working environment.
Good written and verbal communication skills.
Valid CA Driver’s License
Ability to pass a criminal background check
Reliable attendance and punctuality
Familiarity with working with LBGTQ population, youth with Autism, former Foster youth, and youth
within the Juvenile Justice system preferred
