Job Summary

The Structured Day Program Coordinator - Float provides facilitation of services to persons with acquired brain injury by assisting Members to actively participate in the InSPIRE Structured Day Program or to work productively in one of five Clubhouse Work Units (Communications, Community Resources, Culinary & Nutrition, Horticulture, and Maintenance).

Responsibilities

  • As part of the transdisciplinary team, helps design and implement interventions from the Individual Rehab Service Plan that will lead to successful productive daily activity for every Member of the Clubhouse and InSPIRE.
  • Responsible for supervision, safety, and welfare of assigned Members at all times.
  • The Float Coordinator provides coverage for one of the Clubhouse Work Unit Coordinators or InSPIRE Program Coordinators who are absent from programming.
  • Responsible for supervision, safety, and welfare of assigned Members at all times.
  • Responsible for the weekly monitoring and documentation of Members’ activities in the InSPIRE and Clubhouse structured day programs.
  • Responsible for a partial InSPIRE Program Coordinator caseload for documentation (i.e., weekly notes, RSP narratives, other required documentation) and attends and participates in Transdisciplinary Team Meetings for assigned Members, including ITCs, and RSPs.
  • Assists other coordinators develop work and activities for Clubhouse and InSPIRE and evaluates skills necessary for performing tasks, and breaks work down into tasks that each Member can perform.
  • Functions as part of a continuum of services that assures therapeutic goals and objectives will be designed to provide brain injury skills training, assistance and/or supervision to Members in the areas of daily living, personal care, social integration, vocation, and community assimilation utilizing compensatory strategies developed by the Transdisciplinary Rehab Team in accordance with the Member’s RSP.
  • Utilizes cognitive, behavioral, physical rehabilitation, job coaching techniques (especially task analysis) environmental modifications, adaptive equipment (including assistive technology), and compensatory strategies to facilitate Members’ engagement in group and individual activities in order to maximize their independent performances.
  • Facilitates Members’ interpersonal skills to enable positive social interactions, including developing strategies for effective negotiation and healthy conflict resolution, in order to improve quality of relationships in residential, recreational, and work settings.
  • Contributes to the development of an individual’s goals and objectives in their RSP/ISP.
  • Collects data and documents on goals and objectives and gives feedback on performance.
  • Work hours may require evening and weekend availability for time limited periods, to support evening and weekend activities in the Clubhouse and InSPIRE.
  • Coordinates InSPIRE LST’s & Directs Residential LSTs who are assigned to Members, teaching LSTs the strategies each Member needs for support to engage productively in program activities and work, and observes LSTs work with Members.
  • Assists in program evaluation and program development activities to make the program best meet the needs of the Members.
  • Work hours may require evening and weekend availability for time limited periods, to support evening and weekend activities in Clubhouse and InSPIRE.

Job Specific Competencies: 

  • Has knowledge of acquired brain injury and is able to demonstrate effective cognitive and rehabilitation techniques with Members.
  • Demonstrates basic knowledge of licensing and accreditation regulations to assure a safe and healthy environment for all Members.
  • Demonstrates ability to provide a nurturing, positive, individualized approach to each Member to promote their growth and well-being.
  • Demonstrates skill sets necessary to develop individualized goals for the Member in life skills to be able contribute to a productive daily activity pattern.
  • Communicates effectively with Members, family, funders, other staff, support systems and other stakeholders.
  • Facilitates active involvement of Members and their family/support systems in the service delivery process.
  • Demonstrates understanding of, and facilitates developmental/life transitions.
  • Demonstrates knowledge of community resources and seeks to integrate Members in participation in roles in the community.
  • Demonstrates recognition of, and properly reports suspected abuse and neglect.
  • Demonstrates sensitivity to diversity issues and ensures that the culture of each Member is understood and respected by Staff.
  • Demonstrates compassion and respect for the rights of people with disabilities and demonstrates the ability to earn their confidence.
  • Receptive to and demonstrates an open-mindedness to change; proactively works to learn and accept change; accepts unexpected job tasks based on program and/or company needs.
  • Sets and maintains professional boundaries.

Qualifications

Education

  • Bachelor’s Degree in Human Services field or Recreational Therapy. Certification in Brain Injury Specialist (CBIS) Eligible. 
  • 1 to 2 years work experience in human service field required.
  • Brain Injury Experience preferred.

Structured Day Program Coordinator II

  • Proficient in Microsoft Outlook and Office (especially Word, Excel, Publisher, and Power Point).
  • Must become proficient in use of current electronic health record system within 3 to 6 months of hire.
  • Able to lift/transfer a minimum of 50 pounds.
  • Must have own transportation and a valid driver’s license. Proof of valid automobile insurance submitted annually.
  • Must complete Smith Driving Training Course.
  • Must complete and pass ServSafe Certification.
  • Must complete Chapters 1-12 of ACBIS training within 1st year of employment.

Structured Day Program Coordinator III

  • Proficient in Microsoft Outlook and Office (especially Word, Excel, Publisher, and Power Point).
  • Must become proficient in use of current electronic health record system within 3 to 6 months of hire.
  • Able to lift/transfer a minimum of 50 pounds.
  • Must have own transportation and a valid driver’s license. Proof of valid automobile insurance submitted annually.
  • Must complete Smith Driving Training Course.
  • Must complete and pass ServSafe Certification.
  • Must complete Chapters 13-24 of ACBIS training and pass CBIS Certification test and maintain certification within 1st year of employment.
