Winters Healthcare is Hiring a Behavioral Health Clinician Near Winters, CA

Behavioral Health Consultant (LCSW, ACSW, LMFT, AMFT, Ph.D., or Psy.D. )

Basic Spanish is required (such as greeting patients & we can teach this!)- fluent is a plus!


Hours: 24, 32, or 40 hours/week (option of 3, 4, or 5 days)

Location: Winters, & Esparto, CA (24-32 hours/ week in Winters, 8-16 hours/week in Esparto)

$20,000 signing bonus for candidates outside of the Partnership Health Plan (PHP) 24 county service area (provided by PHP) * Associates eligible upon licensure

We are on the lookout for a BHC to join our team of compassionate and dedicated individuals! The BHC functions as a core member of a collaborative care team that involves the patient’s primary care provider, a consulting psychiatrist and other mental health providers that may be available in the primary care clinic. The BHC is responsible for delivering integrated mental health care in consultation with a primary care team, and coordinating referrals to clinically indicated services outside the primary care clinic (e.g. social services, mental health specialty care, and substance abuse treatment). The BHC assists in the assessment and management of behavioral health issues and bio-psycho-socially influenced health conditions in primary care, and uses the Primary Care Behavioral Health Model (PCBH) to provide generalist behavioral health services that are accessible, routine, and flexible.

The ideal candidate enjoys working on a team, making connections, and building relationships. In addition, the ideal candidate will have strong communication skills, – listening included – the ability to establish effective, trusting relationships, and passion to provide exceptional care to the patients of Winters Healthcare as we partner with them in their health journey.

Responsibilities:

  • Provides services as a behavioral health consultant of the primary care or behavioral health team
  • Works with primary care providers to monitor and manage “at-risk” and “chronic” patients
  • Performs assessment and brief treatment for a wide range of psychological and BH needs using brief therapy model
  • Transitions rapidly between patients while maintaining patient care standards
  • Makes appropriate referrals to the county and outside agencies and seeks resources to aid patients
  • Produces professional and accurate written work independently with minimal supervisory correction or editing
  • Develops and maintains interpersonal relationships to be effective team member within a department

Qualifications:

  • Credentials include: CA LCSW, ACSW, LMFT, AMFT, Doctorate in Clinical/Counseling Psychology (accredited programs)
  • Knowledge of psycho-pharmacology and experience working through brief patient contact to make quick and accurate clinical assessments of behavioral concerns
  • Basic Spanish is required (greeting, etc.) Electronic tablet interpreter is available.
  • And last but, certainly not least, we want you to love what you do!

Hours:

  • The position is full time 40 hours a week (1.0 FTE). You will have a consistent schedule each week
  • We are open to candidates interested in less than full-time such as 24 or 32, and option of 3, 4, or 5 days per week)

Compensation and Benefits:

  • Hourly rate range varies on candidate’s credentials and experience:
    • Clinical Psychologist (Ph.D./Psy.D.): Annual equivalent salary range for full-time is $103,924 - $128,492
    • Licensed MFT & Licensed CSW annual equivalent range for full-time is ~$91,520 - $113,360 ($44 - $54.50/ hour)
    • AMFT and ACSW wage ranges from $28 - $35.50 / hour (annual equivalent ~$58,240 - $73, 840)
  • Covered insurances include Medical, dental, vision, life, AD&D – 100% covered for employee; $2,400 flat contribution for dependents
  • Paid Time Off (PTO), floating holidays, and paid holidays off. (~30 days in year 1)
  • Continuing Medical Education (CME): $1,500 annual reimbursement and up to 40 hours CME paid time
  • Licensure reimbursement, and professional liability coverage

Behavioral Health Consultant

Reports to: Medical Department Manager

FLSA Status: Non-exempt, hourly

Summary of Duties
The Behavioral Health Consultant (BHC) functions as a core member of the integrated care team, assisting in the assessment and management of behavioral health issues and bio-psycho-socially influenced health conditions in primary care. The BHC will utilize the Primary Care Behavioral Health Model (PCBH) to provide generalist behavioral health services that are accessible, routine, and flexible. The BHC will provide direct clinical services to patients in person, and/or by phone or video as needed and will also assist with assessments and referral to traditional behavioral health services (such as mental health specialty care and substance abuse treatment) as needed.

Essential Functions:

  • Provides services as a behavioral health consultant of the primary care or behavioral health team
  • Works with primary care providers to monitor and manage “at-risk” and “chronic” patients
  • Performs assessment and brief treatment for a wide range of psychological and behavioral health needs using brief therapy model as first line intervention
  • Provides simultaneous focus on health and behavioral health issues
  • Manages time to meet the needs of patients, provide immediate assistance or referral as needed, and have knowledge of service areas
  • Transitions rapidly between patients while maintaining patient care standards
  • Develops treatment plans based on assessment information that are documented appropriately including identified goal(s), behavioral/psychological problem statement, measurable objectives, and estimated time frames for achievement
  • Provides effective triage of patients in need of specialty behavioral health and psychiatry
  • Makes appropriate referrals to the county and outside agencies and seeks resources to aid patients
  • Writes patient notes clearly in order to provide adequate information for other health care providers
  • Takes responsibility for all work activities and personal actions by following through on commitments
  • Obtains complete and accurate information in a courteous, respectful, and culturally sensitive manner in order to determine a diagnosis and appropriate treatment plan
  • Evaluates suicidal individuals or individuals in crisis and manages their safe disposition
  • Utilizes assessment information and collateral information to conceptualize patient psychopathology
  • Utilizes the DSM and assessment information to accurately and routinely diagnose individuals treated
  • Produces professional and accurate written work independently with minimal supervisory correction or editing
  • Provide walk-in behavioral health services as needed.
  • Develops and maintains interpersonal relationships to be effective team member within a department which is focused on team building
  • Aids primary care providers by answering questions regarding patient care
  • Adheres to agency policy regarding documentation and medical records procedures
  • Maintains documentation in compliance with State, Federal, Professional, and Ethical guidelines
  • Work within a fast-paced primary care setting: provide assessment, consultation, and brief intervention for psychological/psychiatric concerns and/or disorders.
  • Support performance and quality improvement efforts, and participate in related activities as required.
  • Performs other duties as assigned


Minimum Requirements:

Education and Work Experience

  • Current California license to practice as a LCSW, ACSW, LMFT, AMFT, or Doctorate in Clinical/Counseling Psychology (accredited programs)
  • Experience working through brief patient contact to make quick and accurate clinical assessments of behavioral concerns; experience in primary care, and community health center setting preferred,
  • BCLS, ACLS, or CPR Certification (or receipt of certification within 60 days of hire)

Knowledge, Skills and Abilities:

  • Fluency (the ability to read, write and speak) in English and Spanish is preferred
  • Ability to effectively engage patients in a therapeutic relationship
  • Ability to effectively communicate with primary medical care providers about mental health issues treatment.
  • Ability to provide simultaneous focus on health and behavioral health issues
  • Experience with screening and assessment for common mental/substance use disorders.
  • Working knowledge of evidence-based psychosocial treatments for common mental disorders.
  • Working knowledge and experience with brief, structured counseling techniques (e.g. Motivational Interviewing, Behavioral Activation).
  • Knowledge of psycho-pharmacology
  • Well-organized and detail-oriented with the ability to establish and maintain working relationships with patients, employees and the public
  • Experience operating standard office equipment
  • Experienced with computer systems (eClinicalWorks) in order to obtain patient information, document clinical data, notes, and check messages
  • Requires professional and effective oral and written communication skills, excellent interpersonal skills, and conflict resolution skills
  • Emotional intelligence and critical thinking skills
  • Ability and comfort building relationships with coworkers and patients
  • Demonstrated team experience and proven successes in implementation of team goals
  • Significant background providing outstanding customer service
  • Proficient in computer programs such as Microsoft Office: Word, Excel, Outlook
  • Applies knowledge of the Privacy Act of 1974 and the Health Insurance Portability and Accountability Act of 1996 (HIPAA) to ensure the privacy of individuals is protected to the fullest
  • A passion and dedication for serving the healthcare needs of our clients

Other Requirements:

  • Authorization and consent for Winters Healthcare to investigate candidate’s background with a consumer report for employment purposes, to evaluate candidate/employee for employment, promotion, reassignment, or retention as an employee

Additional Desired Qualifications:

  • Experience with Electronic Health Records (EHR) preferably eClinicalWorks (ECW)
  • Previous experience in a health center setting is highly desirable.
  • Experience working with an integrated behavioral health team.

While performing the duties and tasks of this job, Employee is regularly required to stand; walk; work on irregular surfaces; reach with hands and arms; use hands to finger, handle or feel objects, tools, or controls; talk or hear; and taste or smell. Employee is occasionally required to sit and stoop, kneel, crouch, bend, squat, twist or crawl. This job requires physical effort and the ability to place or retrieve items at below waist level may be required.

Employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision and depth perception, and the ability to adjust focus. Employee occasionally may be required to transfer patients to/from a wheelchair. The physical demands described here are representative of those that must be met by Employee to successfully perform the essential functions of this job. Environmental/ Working Conditions:

The environment is a standard medical office environment. Regular office hours are Monday through Friday. Occasionally, evenings, weekends and overtime may be required.
