The Care Manager works under the supervision of the Administrator of County Services and the general direction of the Deputy Director. The Care Manager performs professional assessments of an individual’s capabilities and the family’s capacity to provide care. They are responsible for coordinating, monitoring client status, needs, changes, reassessments and complaints. Come join the WRAAA team!!!!

Requirements

• BA or BS in Related Field
• LPN Degree Eligible
• Experience in Care Management will also be considered.
• Social Services type Degree
