White Oak Healthcare is Hiring an Activities Assistant- Weekends Near Fremont, NH

SUMMARY

Responsible to designing, planning, and leading recreation programs in our short-term stay unit, long-term care unit, and assisted living community as assigned by the Director of Activities. Aide is responsible for completing required documentation including assessments, progress notes, and attendance of meetings, individualized activity interventions, community outings, and other duties as assigned by the director.

RESPONSIBILITIES AND STANDARDS:

1.Communicates to necessary clients, residents, and staff all changes in programs.

2.Assists in preparing and distributing resident/client’s monthly calendars.

3.Insures the safety of residents when planning activities and transporting residents to activities, including community outings.

4.Responsible for resident/client safety during activities.

5.Completes resident quarterly progress notes, initial assessments, and attends care plans as assigned by the Activities Director. (MDS Coordinator or Director of Social Services may delegate assessments/invite to scheduled care plan meetings in the absence of the Activity Director.)

6.Completes one-on-one room visits as assigned.

7.Responsible for care, maintenance and storage of activity supplies and activities storage closet.

8.Observes, reports, documents, incidents of resident behavior condition at activities to the Activities Director.

9.Completes resident accident/incident forms as needed at the discretion of the Activities Director/Director of Nursing.

10.Attends staff meetings and mandatory in-services.

11.Assists in resident programming and special events.

12.Participates in special events that may include weekends, nights or holidays as directed by Activities Director.

13.Performs other duties as assigned by the Director of Activities.

14.Responsible in projecting a personal and professional image when dealing with people external to the facility.

15.Demonstrates pride in a job well done.

16.Contributes to team effort by accomplishing related results as needed, providing instruction in areas of expertise to other team members when necessary as part of the on-going educational process.

17.Is able to work independently as well as collaborate with other department members.

18.Completes resident assessments according to facility policy in a timely manner.

EDUCATION /EXPERIENCE:

·High school graduate

·One year experience in long term care or related field preferred

·Experience in leading social/recreational groups and comfortable speaking in front of groups of people

·Knowledge of both the physical and psycho/social process of aging preferable
