Coordinator of Care, Case Manager - Home Care - Register Nurse (RN) - - Field based

Regional coverage needs: Lower Manhattan, Queens and Brooklyn

(Car and driver license required for Queens and Brooklyn regions)

Position Overview:

You will play a pivotal role in coordinating, implementing, and evaluating the home care needs of patients. Your responsibilities will include reviewing patient insurance information, coordinating activities within individual care plans, and ensuring relevant findings are communicated effectively with family and professional caregivers. Your strong communication, documentation, and time management skills will be crucial for success in this role. You will enjoy a high level of autonomy and respect for your opinions as you navigate the complexities of patient care.

Qualifications:

• Education: ASN Required, BSN preferred.
• Experience: Minimum of one-year acute Medical-Surgical nursing experience; CHHA/LTHHCP experience preferred.
• Patient care experience Individuals aged 60 and Adult
• Homecare experience
• Specialized Knowledge and Skills: Knowledge of Microsoft Office and experience documenting in a clinical software system preferred. A driver’s license is highly preferred.

Responsibilities:

• Create and update comprehensive and interdisciplinary patient-centered home health plans of care in collaboration with physicians.
• Case manage care and services for assigned patients to achieve positive clinical outcomes and minimize risks of rehospitalization.
• Support agency-established financial goals by accurately completing OASIS data sets, entering physician orders into the EMR, and providing timely documentation for reimbursement.
• Demonstrate expert knowledge unique to the practice of community health nursing by adhering to agency guidelines and state and federal regulations.

Pay: $100,000.00 - $115,000.00 per year
