Assisted Home Care Services is looking to hire a full time Home Care Coordinator. Candidate must be able to work in a fast paced environment and possess excellent organization and communication skills.

Job Description:

• Complete introductory calls to all new patients.
• Schedule caregivers with clients ensuring compatibility and customer satisfaction.
• Monitor the attendance call in & out/presence of employees in the home on a routine basis.
• Review & Run Missed Ins, Missed Outs, Call Maintenance, and Pre-Billing reports to ensure aide attendance.
• Responsible to run the open orders to make certain all cases are covered.
• Maintain open means of communication with field staff to identify and document changes in schedule, patient's health status, & additional information as needed.
• Enter and maintain accurate client billing and employee payroll records in tracking system (HHA Exchange).
• Maintains patients files and a healthy relationship with the patient, patients' families, and field staff.

Qualifications:

• Knowledge of HHA Exchange preferred.
• Must be organized and detail oriented.
• Excellent communication skills.
• Ability to work independently and make decisions.
• Home Care experience preferred.
• Bilingual Mandarin and Cantonese.
