GENERAL JOB DESCRIPTION
Implements high quality physical therapy services in accordance with state and federal regulations, professional principles and practices, and facility policies and procedures. Supervises Rehabilitation Techs / Aides.
DUTIES & RESPONSIBILITIES
  • Performs screening and provides quality treatment according to an established plan of care under the supervision of a physical therapist for all patients that could benefit from intervention.
  • Ensures that all residents receive care in a manner that attains or maintains the highest practical mental and physical functional status and quality of life.
  • Adheres to the company’s policies and procedures, state and federal regulations, professional standards and ethics, and Resident Rights.
  • Adheres to state practice act and professional guidelines regarding required physical therapy supervision practices.
  • Documents accurately, effectively and timely each patient’s progress towards developed goals.
  • Utilizes good communication and interpersonal skills in communicating a patient’s status and needs to appropriate individuals.
  • Provides in-services and training to the patient, family/caregivers and facility staff as needed or assigned.
  • Provides supervision or mentorship to rehab aides/techs to facilitate continued development of their skills during assigned tasks or assisting with patient treatment.
  • Actively contributes to interdisciplinary patient care through attendance and participation in case management meetings and referral to other disciplines as appropriate.
  • Attends department and facility weekly meetings and supports/participates in clinical programs and performance improvement projects as assigned.
  • Demonstrates knowledge of the company’s vision and values and displays behavior promoting clinical excellence, customer-centered care and the belief that we treat people the way we want to be treated.
  • Meets the facility policy on attendance, punctuality and dress code; demonstrates flexibility regarding work schedules including holidays and weekends to meet patient care needs.
  • Meets the department’s productivity standards and budgetary expectations by exercising time management skills and efficient coordination of care.
  • Ensures strong customer relations within the facility and the community.
  • Maintains clinical competency and pursues professional development. Seeks input/feedback from others and actively takes corrective action to continuously work on self-development and improvement.
  • Embraces the team concept by demonstrating encouragement and support to peers and maintaining a good working relationship with colleagues, promoting harmony and a positive work environment in the facility.
  • Demonstrates effective clinical problem-solving skills and a solution-oriented approach to identify and prioritize issues in a responsive manner, developing sound and innovative action plans, and executing action plans on a timely basis to ensure appropriate results.
  • Other duties as deemed necessary and appropriate, or as may be directed by the Director of Rehabilitation.
EDUCATION & TRAINING
Must possess a valid state license as a Physical Therapy Assistant.
KNOWLEDGE & EXPERIENCE
A minimum of 1 year of clinical experience with the adult patient population preferred, but not required.
SKILLS & ABILITIES
  • Ability to lift patients (capable of easily lifting 50 lbs. of dead weight alone)
  • Ability to walk, bend, stand and reach constantly during a work day
  • Visual, auditory, and oral skills sufficient to evaluate, diagnose, communicate and monitor patient needs, and to maintain accurate records, recognize people and provide and understand written, verbal and gestural communications
  • Ability to push wheeled equipment throughout the facility and the community
  • Fine motor skills for legible and accurate writing; and computer generation of reports, charting, scheduling, daily correspondence and presentations
  • Fine motor skills for effective and efficient handling of diagnostic or therapeutic equipment
  • Ability to participate in sustained activity for many hours in accordance with the state labor laws
  • Must be able to cope with the mental and emotional stress of the position
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies
  • Must meet the general health requirements set forth by the policies of this facility which include a pre-employment medical and physical examination
  • Must be able to relate to and work with ill, disabled, elderly, and emotionally upset people within the facility
  • Assist as necessary in the evacuation of residents during emergency situations
