As a Home Health Therapy Coordinator, you will play a crucial role in ensuring the efficient and effective delivery of therapy services to our clients within their homes. You will coordinate therapy services, manage schedules and invoices, and facilitate communication between therapists, clients, and other healthcare professionals involved in the care process. This role requires exceptional organizational skills, communication abilities, and a commitment to delivering high-quality care to our clients.

Responsibilities:

  1. Therapy Service Coordination:
    • Coordinate therapy services, including physical therapy, occupational therapy, and speech therapy for home health care clients.
    • Schedule therapy appointments based on client needs, therapist availability, and prescribed treatment plans.
    • Ensure timely delivery of therapy services and adherence to treatment schedules.
  2. Client Communication and Support:
    • Serve as the primary point of contact for clients receiving therapy services at home.
    • Communicate effectively with clients and their families regarding therapy appointments, treatment plans, and any changes in scheduling.
    • Address client inquiries, concerns, and requests in a prompt and professional manner.
  3. Therapist Support and Collaboration:
    • Coordinate with therapists to ensure they have the necessary information and resources for each client visit.
    • Facilitate communication between therapists, clients, and other healthcare professionals involved in the care team.
    • Provide administrative support to therapists, including managing documentation and reporting requirements.
  4. Documentation and Record-Keeping:
    • Maintain accurate records of therapy appointments, client progress, and treatment plans.
    • Ensure compliance with documentation standards and regulatory requirements for home health therapy services.
    • Assist with billing and reimbursement processes related to therapy services.
  5. Quality Assurance:
    • Monitor the quality and effectiveness of therapy services delivered to clients.
    • Identify opportunities for improvement in service delivery and collaborate with the management team to implement changes.
    • Participate in quality assurance activities and performance improvement initiatives.
Requirements:
  • Prior experience in home healthcare.
  • Strong organizational skills with the ability to prioritize tasks and manage multiple responsibilities simultaneously.
  • Excellent communication and interpersonal skills, with the ability to interact effectively with clients, therapists, and other healthcare professionals.
  • Proficiency in computer applications and electronic health record systems.
  • Knowledge of healthcare regulations and compliance requirements related to home health therapy services.
  • Compassionate and empathetic attitude towards clients and their families.
  • Ability to work independently with minimal supervision and as part of a multidisciplinary team.
