JOB SUMMARY:
Community Health Workers will work with specific target populations of identified members of the community. The CHW's will work closely with (MCO or health system), referral agencies, medical providers, primary care teams, and other agencies to improve the member’s care and outcomes. The Community Health Worker will serve as a liaison/connector between the member, care team, and the community. The CHW will help to bridge conversations with members and remove barriers that prevent them from accessing health and social services.
JOB DUTIES:
1. Develops trusting relationship with members by providing support and advocacy to help achieve health goals.
2.Convey the purposes and services of the CHW to the member and impact that program or service would provide
3.Meets member in clinic, facility, office, or at home to help identify social determinants of health affecting member’s health and general well-being.
4.Help members develop health management plans and goals
5.Follow-up with health management/care plans with both members and providers
6.Coach members in effective management of their chronic health conditions and self-care
7. Assist member in understanding care plans and instructions
8. Document activities, service plans, and results in an effective manner
9.Work collaboratively and effectively within a team
10.Establish positive, supportive relationships with members and provide feedback
11. Help members in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible
12.Assist members in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services
13.Facilitate communication and coordinate services between providers
14.Motivate members to be active, engaged participants in their health
15.Effectively work with people (staff, members, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between members and institutions
16.Build and maintain positive working relationships with the members, providers, nurse case managers, (MCO or health system) representatives, supervisors and office staff
17.Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations
18.Make frequent visits to individual homes and community organizations.
19.Other duties as assigned
JOB SPECIFICATIONS:
A passion for people who are under served.
A minimum on one years’ experience working in health/social services
Education above high school diploma or GED.
Must be a resident of the community.
Ability to be sensitive and responsive to diversity, equity and inclusion.
Ability to communicate effectively with clients, co-workers, volunteers, and supervisor.
Have flexible and creative attitude and approach
Candidate must pass a background check, Drug Screen, and take TB test
OTHER REQUIREMENTS:
Must have a valid Driver’s License with good driving record
Liability insurance that meets minimal requirements
Vehicle must display current safety inspection sticker
Criminal background will be checked
Must have a cell phone
