As a Licensed Practical Nurse - LPN within our Care Management team, your day is a dynamic mix of patient care and coordination. You work closely with the Medical Provider (MD and NP), supporting their schedule and preparing clients for appointments. Your role encompasses various integrated care functions, including liaising with external providers, managing medical scheduling and reminders, coordinating comprehensive care, facilitating referrals to social support systems, and ensuring quality measures are met. Additionally, you handle medical tasks like routine tests, medication administration, vital sign recording, and lab management, all while maintaining meticulous documentation and a culturally sensitive approach to patient care. You may also engage in activities such as medication phone calls, prior approvals, managing abnormal lab results, and inventory management. Your dedication to delivering exceptional healthcare services through versatile responsibilities is crucial to our mission.

ARE YOU OUR IDEAL LICENSED PRACTICAL NURSE - LPN?

• Completion of a state-approved certified nursing program
• Successful passing of a competency program that meets state regulations
• Unrestricted certification as an LPN
• 6 months of experience as an LPN in a healthcare/psychiatric setting
• Licensed Practical Nurse with a Tennessee license
• Experience in an office setting

Experience working with psychiatric clients is preferred but not required.
