HOMELESS OUTREACH CASE MANAGER (PATH Program):
Immediate part-time opening at our Cookeville Campus.
This position is responsible for obtaining program resources, all reports/record keeping, assessment to determine eligibility, and paperwork involved with the PATH Grant. The program focuses on working with adults who are mentally ill and homeless, providing daily living assistance, and referrals to community resources, advocacy and transition to mental health services.
QUALIFICATIONS:
A bachelor's degree in psychology, Social Work, or related area is required.
Experience working with the homeless preferred and must have an understanding of homelessness.
