EXEMPLARY DUTIES / RESPONSIBILITIES:
The Care Coordinator provides targeted support services, via the wraparound process, to families with high risk young children/children/youth/transition age youth, regarding behavioral or emotional problems, parenting issues, and life stressors by developing and implementing bio-psycho-social assessments and service plans with the family and wraparound team members. The Care Coordinator utilizing the System of Care (SOC) philosophy will work with families, social service organizations, and other state agencies to develop a coordinated network of resources and supports in their communities.
· Works closely with Program Director regarding all grant program protocol and data collection instruments. Must be willing to have a flexible schedule to meet the needs of the client with possible weekends, evenings or early mornings.
· Responsible for ensuring all phases and activities of the Wraparound Process are done to high quality and fidelity – accountable to family team.
· Provides outreach and engagement to families upon referral to the program by conducting initial phone contact and the intake process.
· Completes an assessment of the family including Child and Adolescent Needs and Strengths (CANS) and other national evaluation tools to develop a care plan.
· Provides face-to-face direct service to families including support, referrals and assistance with the development of positive parenting, safety planning, basic life skills and parenting a child/youth with special needs.
· In collaboration with the FSS (if involved), assists family in developing a list of individuals they want to attend the Family Team Meetings.
· Ensures that the wraparound process is family driven and that families experience ongoing participation in and ownership of their care plans.
· Provides liaison functions for the team members and service providers.
· Creation of the family team and scheduling of case conferences/family team meetings in collaboration with family members.
· As a result of the family team process, develops, and coordinates Individual Service Plan (ISP) with the family.
· Linkage to community resources, clinical services, and natural supports related to the child/youth.
· Available for crisis calls; collaborates with clinician/therapist to provide support during the crisis and following the crisis.
· In consultation with therapist, ensures that crisis and safety plan and assessment are completed.
· Participates in team-based supervision meetings.
· Skills building for the child/youth as appropriate.
· Team maintenance and transition coordination.
· Maintains up-to-date case records on each client family, including weekly contact reports, service agreements, evaluations, and termination summaries.
· Maintains and builds community relations, through education and advocacy and participating in external meetings such as the Community Action Board (CAB), Council on Children’s Mental Health (CCMH) meeting and other meetings related to children/youth and families.
· Collaborates with program supervisor to train staff; provides field case consultation & support to new staff.
· Participates and engages in the local SOCAT team.
· Works with the SOCAT Regional Coordinator to identify and engage with local community resources.
· Provide SOC 101, wraparound orientation and training, and other pertinent trainings to new staff and other community partners.
· Other duties and tasks as assigned.

· Provide a continued focus on goal setting and goal attainment.
· Monitor and support team members to complete assigned actions steps.
· Knowledge of children and youth who have emotional and behavioral disorders.
· Knowledge and experience with the juvenile justice, educational, and/or other child-serving systems.
· Desire to help and support families, with strong customer service skills.
· Commitment to system of care principles, cultural competency & effective team work.
· Basic computer and writing skills.
EDUCATIONAL/ EXPERIENCE:
A bachelor’s degree in a social or behavioral science or other related acceptable field with one to three years’ experience or training in social services. Experience working with children/youth and families with significant behavioral health needs. Ability to provide a strength-based, client-centered approach to service delivery. Ability to work remotely.
