Licensed Nurse Practitioner - Mental Health - West Palm Beach, Florida

Duties include but are not limited to:

• Provides case management for SSVF grantees who are homeless and experiencing substance use disorder (SUD) concerns
• Help to eliminate homelessness and to increase access to treatment by addressing unmet health and social needs which may be impacting participation in treatment.
• Work with homeless program staff to help the Veterans with SUD engage in VA treatment services including but not limited to SUD specialty care or residential treatment
• Assesses the physiological and psychosocial health status of the SSVF Veterans and makes appropriate referrals for treatment
• Accountable for adhering to established policies, standards of care and standards of practice. to be responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care
• Assesses, plans, implements, and evaluates care based on age specific components
• Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction
• Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences
• Administers medications and procedures per established policies and guidelines
• Influences care outcomes by collaborating with members of the interdisciplinary team
• Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care
