Visiting Nurse Association of Hanover & Spring Grove is Hiring an Occupational Therapist Near Hanover, PA

The Occupational Therapist OT provides occupational therapy for home health patients in accordance with the individual patient’s needs and goals as addressed in the plan of care. The OT must have knowledge and understanding of a patient’s illness or injury and the ability to assess these needs and plan appropriate interventions. The role of the occupational therapist is to work as part of the home care team including RN, PT, SLP, MSW and Home Health/Hospice Aides to assist the patient in achieving their optimum goals. The Occupational Therapist also provides palliative occupational therapy services to VNA Hospice patients on an as-needed basis, in accordance with the hospice plan of care.

Occupational Therapy Services 

  1. Evaluate patient’s functional status, muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc. 
  2. Evaluate home environment for hazards or barriers to independent living.
  3. Participate in the development of and communicate the plan of care to attending physician, home care team members and patient/family and/or caregivers as it relates to occupational therapy services.
  4. Provide occupational therapy services to the hospice patient relative to the activities of daily living to devise alternative ways of accomplishing these tasks in order to allow the patient to remain more independent (and less dependent upon others). This may include working with adaptive or assistive devices or explaining energy conservation techniques to enhance the patient’s quality of life.
  5. Teach patient and/or family/caregivers' new skills or retrain in activities of daily living, in accordance with the plan of care. 
  6. Fabricate splints as needed and instructs patient, families and/or caregivers in the use of various types of adaptive equipment to improve function. 
  7. Train patient, family and/or caregiver in the use of prosthetic and/or orthotic devices.
  8. Report patient’s reaction to treatment or changes in care to patient’s physician as needed and appropriate.
  9. Instruct and educate other personnel, patient and/or family/caregivers in patient’s therapy treatment regime, as well as the goals of therapy. 
  10. Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care. 
  11. Maintain appropriate clinical records, progress notes and reports to the physician and submits these records in a timely manner in accordance with VNA of Hanover and Spring Grove policies and procedures, Medicare Conditions of Participation for Home Health and Hospice, licensure laws, and any related state and federal regulations. 
  12. Instruct patient and/or family/caregiver and other team members in the patient’s treatment regime as indicated. 

Qualifications:

1. Graduate of an occupational therapy curriculum accredited jointly by the committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.

2. Minimum of two years clinical experience as an Occupational Therapist. Community/home health experience preferred. 

3. Current license as an Occupational Therapist from the Commonwealth of Pennsylvania or made application for same.

4. Must possess valid PA driver’s license.

5. Must have the use of an automobile in good working condition and automobile insurance.

6. CPR certified.

7. Satisfactory references from employees and/or professional peers.

8. Satisfactory criminal background check.
