The Emergency Department Clinical Social Worker 2 delivers discharge planning and social work services during normal business hours and the entire hospital after hours. This job requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric. This job provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services. Responsible for the daily management of all complex discharge planning and social work functions.Licensure, Certification, or Registration Requirements for Hire: Licensed Clinical Social Worker in the State of Virginia preferredLicensure, Certification, or Registration Requirements for continued employment: N/AExperience REQUIRED: Minimum of one (1) year of clinical social work experienceExperience PREFERRED Clinical social work experience in an acute care setting Related experience in acute discharge planning, home health, case management, long term care or related community agencyEducation/training REQUIRED: Masters of Social Work Degree from a Council on Social Work Education accredited programEducation/training PREFERRED: N/AIndependent action(s) required: Primary responsibilities include proactive development, implementation and ongoing revision of an interdisciplinary plan of care for patients who receive care in the Emergency Department. The Emergency Department Clinical Social Workerdelivers discharge planning and social work services during normal business hours and the entire hospital after hours. This position requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric.
The Emergency Department Clinical Social Worker provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals, and crisis intervention services. The position is responsible for the daily management of all complex discharge planning and social work functions.

This position will also conduct clinical biopsychosocial assessments to include mental health symptoms, substance use issues, housing concerns, financial and employment problems, abuse and domestic violence history, religious faith or spirituality, military status, medical issues, and any other psychiatric, social, or health issues. They will collaborate daily with patients, their families, the interdisciplinary team, and community agencies to facilitate a safe discharge or transition to appropriate level of care.
