We are seeking a Care Manager Associate to join our growing Care Management team who provide families, and vulnerable populations with medical and therapeutic support to live their best life while coping with chronic, acute, or terminal illnesses.

Position Summary:
The primary role of the Care Manager Associate is to support the Care Management team by assisting with client care coordination, transporting clients to medical appointments, advocacy, and clinical insight into care needs.
Care Management services will be provided in home, telephonically and/or video.
You will plan and manage your daily schedule to provide high quality care that is client-centered and timely. This is a Monday-Friday role
