Description

The Field Case Manager I, you have the opportunity to be part of the solution to help our homeless neighbors living on the streets within King County. This position will directly support the Outreach Ministries' efforts to end homelessness one person at a time by providing personal engagement, key resources, and the opportunity to share the Good News and the UGM recovery programs.

Shift

Monday – Friday 9:00 am to 5:30pm.

What you will do 

  • Be a Light of Christ to our Homeless neighbors living on the street in King County in praying with them and sharing the Gospel. Research & develop outreach strategies to homeless 
  • Process intake assessments and screening of individuals ministered to during outreach.
  • Manage caseload of individuals.
  • Provide crisis intervention information and referral help.
  • Work in close partnership with church connections and local service providers to support our homeless neighbors. 
  • Complete End of Month Reports with accurate statistics.
  • Communicate necessary information with Outreach Supervisor.
  • Offer shelter options and coordinate homeless guest intakes and transportation to appropriate shelter/recovery programs. 
  • Drive between various encampment locations within King County and follow-up with homeless guests.
  • Prepare and stock Search and Rescue Van’s with seasonal items such as blankets, food, clothing and hygiene items to distribute to guest in need.
  • Have an understanding and working knowledge of local key resources directly related to homelessness in King County.
  • Coordinate nightly registered volunteers utilizing VHUB volunteer system.
  • Build volunteer relationships and community to inspire and multiply impact to those we serve.
  • Prepare nightly and monthly statistical/narrative reports.
  • Maintain and record timely records of guest interactions and CARE plans within Clarity database.
  • Work closely with the Outreach Administrator to accomplish goals set for Search and Rescue.
  • Attend all Mission meetings and training sessions, as required.
  • Participate in awareness campaigns with Marketing Dept.
  • Any other job-related duties as assigned by supervisor.

Qualifications


  • AAS/BA/BS in Social Service or other behavioral science preferred.
  • 2 years of experience working in a social services setting (social welfare, human services, addition services, etc.) is highly preferred.
  • Proficiency in Microsoft Office and Web-based programs. HMIS training a plus.
  • CPR/First Aid Training Certification and HMIS Training Certification within 6 months of hire.
  • Must possess a valid unrestricted Washington State Driver’s License and be able to meet the standards of the Mission driver criteria required, including but not limited to:
    • Clean driving record
    • No current suspensions or revocations in the last 3 years
    • At least 21 years of age

Work Environment

While performing the duties of this job, the employee is frequently required to stand, walk or sit for extended periods of time; bend, stoop or reach. Frequently exposed to varying indoor and outdoor temperatures and weather conditions. Occasionally will stand, balance, twist, stoop, kneel and reach with hands/arms. Regularly lifts up to 25 pounds. Specific vision abilities required by this job include close vision, color vision, distance vision, depth perception, and ability to adjust focus.
