You will be responsible for the Assessment and Care Plan of assigned Patients.
Responsibilities:
- Assessment of Patients
- Develop Care Plan for assigned Patients
- Quarterly Nurse monitoring visit
- Quarterly Report on changes in patient symptoms or behavior for Monitored Patients
- Maintain accurate patient medical records
Qualifications:
- Previous experience in nursing or other medical fields
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
