Registered Nurses (RN)/Licensed Practical Nurse (LPN) to help drive the quality care for our residents.

Our Nursing positions need to be caring and compassionate individuals who can provide a strong emphasis on rehabilitation and long-term nursing services. The qualified candidates are responsible for the delivery of patient care through the long-term and skilled care process of assessment, planning, implementation, and evaluation based on established clinical practice standards. Collaborates with other disciplines to ensure effective and efficient patient care delivery for the achievement of desired patient outcomes. Via Health Services has two facilities located on the Southside of Des Moines and in Carlisle serving the long-term and skilled nursing needs for the greater Des Moines area. The Carlisle and Fleur Heights Centers for Wellness and Rehab both support between 80 to 89 beds. In addition, Carlisle's campus has additional homes for Assisted Living and Independent living.

Responsibilities:

• Responsible for the delivery of nursing care and supportive services within the scope of the nurse's education and experience.

Wage:

• Based upon experience.

Qualifications:

• Must have a valid State of Iowa RN license.
• A minimum of 2 years’ experience in long term care and skilled care.
• Experience with working in a CCDI unit a plus.
• Current BLS card.
• Charge Nurse: 1 year (Preferred)

Standard shift:

• Night shift

Supplemental schedule:

• Holidays
• Overtime

Weekly schedule:

• Monday to Friday
• Rotating weekends
• Weekend availability
