Job Summary:
We are seeking a dedicated and compassionate Mental Health Case Manager to join our team. As a Mental Health Case Manager, you will be responsible for providing support and guidance to individuals with mental health conditions. Your role will involve coordinating care, conducting assessments, and developing treatment plans to ensure the well-being of our clients.

Responsibilities:
- Conduct comprehensive assessments to determine the needs and goals of clients
- Develop individualized treatment plans based on assessment findings
- Coordinate and monitor the delivery of services to ensure client needs are met
- Provide crisis intervention and support during emergency situations
- Collaborate with healthcare professionals, including psychiatrists, psychologists, and social workers, to develop comprehensive care plans
- Facilitate group therapy sessions to promote social interaction and emotional support
- Provide clinical counseling and psychotherapy to clients as needed
- Assist clients in accessing community resources and support services
- Monitor client progress and adjust treatment plans as necessary
- Maintain accurate and up-to-date documentation of client interactions

Requirements:
- Bachelor's or Master's degree in Psychology, Social Work, Counseling, or a related field
- Previous experience in mental health case management or a similar role is preferred
- Knowledge of senior care, nursing home environments, and working with individuals with complex needs is highly desirable
- Strong crisis management skills and the ability to remain calm under pressure
- Proficiency in conducting diagnostic evaluations and developing treatment plans
- Familiarity with evidence-based therapeutic techniques such as applied behavior analysis (ABA) or cognitive-behavioral therapy (CBT)
- Understanding of PTSD care and trauma-informed approaches
- Excellent communication skills, both written and verbal
- Ability to work collaboratively within a multidisciplinary team
