Join a team that can truly make an impact on the lives of others! At Health Connect America, we are dedicated to making connections happen every day by cultivating growth, belonging, and wellness for our teammates. What will I be doing?-In this position you will help ensure that client’s are consistently receiving medical care, behavioral health services, and access available resources within their communities. -Contact clients 2-4 times monthly to ensure quality and efficacy of services rendered.-Will report regularly to leadership to receive support and guidance in providing needed services. Experience with case management and work with children and families preferred.
