What will I be doing?-In this position you will help ensure that client’s are consistently receiving medical care, behavioral health services, and access available resources within their communities. -Contact clients 2-4 times monthly to ensure quality and efficacy of services rendered.-Will report regularly to leadership to receive support and guidance in providing needed services. Am I a good fit?-Are you a team player looking to work in a supportive and growth-oriented workplace? -Are you passionate about helping client’s overcome barriers to living higher quality lives?-Do you have the ability to comfortably engage with varied populations in the community via telehealth/phone calls as well as in face-to-face communication?-Are you organized, motivated, and task oriented?Qualifications:-Bachelor’s degree required with preference to human service fields or other health related discipline. Experience with case management and work with children and families preferred.
