Position Description

Job Title: Support Coordinator

Responsible to: Quality Assurance Mentor

Job Summary: The Support Coordinator must help eligible persons and their families to identify, locate and access all proposed Medicaid Waiver funded services, other necessary services, and informal community supports. The Support Coordinator must assist individuals and families in finding and gaining access to necessary supports and services, coordinate the delivery of supports and services and monitor the delivery of supports and services to determine the extent to which they meet the needs and expectations of the individual, family and others who participated in the development of the Support Plan.

Work Activities:

  • Meet at least annually with the customer and others of the customer’s choice, to assist them in defining the supports and services necessary for the customer to work toward his/her vision.
  • Accurately reflect the vision, outcomes and action steps agreed upon in written form (the Individual Support Plan) within 5 working days of the meeting.
  • Assist the individual and circle of support to locate and access the supports and services desired and needed.
  • Complete all required visits and maintain adequate documentation, including support notes that meet the requirements of the Medicaid Waiver.
  • Complete all necessary paperwork to ensure continued eligibility of Waiver funding for each customer.
  • Coordinate and monitor the delivery of paid and unpaid services and supports specified in the Individual Support Plan.
  • Maintain open communication with and between the customer, his/her family or guardian, advocates, staff of provider agencies and other members of the person’s circle of support.
  • Treat the customer, his/her family or guardian, advocates, staff of provider agencies and coworkers with dignity and respect.
  • All other duties as assigned by the Quality Assurance Mentor.

Job Context:

The Independent Support Coordinator spends the majority of the time interacting with the customer, his or her family and other involved in the delivery of services and supports. The position requires travel to and from the homes of customers, homes of the family members and other locations where individuals spend their time. Not all locations will be wheel-chair accessible and the position requires the ability to move on different terrains. The Independent Support Coordinator works from his or her own home for a majority of activities, utilizing computer technology, but must also travel to required training and meetings on a regular basis.

Qualifications:

Minimum:

  • A Bachelor’s degree from and accredited college or university in one of the behavioral sciences
  • A Bachelor’s degree in an alternative discipline and two (2) years of relevant experience in the field of developmental disabilities; or
  • Two (2) year degree plus three (3) years of relevant experience in the field of developmental disabilities; or
  • Five (5) years of relevant experience in the field of developmental disabilities.

Desired:

  • Prefer experience in social work duties and exposure to requirements of MH/MR requirements, licensure regulations and Medicaid Waiver
  • Prefer expertise in computer technology, including word processing and e-mail via the Internet
