WHAT WE OFFER
The County of Ventura offers an attractive compensation and benefits package that includes:
  • A general salary increase of 3.5% effective December 22, 2024.
  • Additional Market-Based Premium Pay (MBPP): Community Health Workers, Whole Person Care, shall be paid the additional hourly pay of two dollars and fifty cents ($2.50) per hour. (Subject to the applicable section of the SEIU 2022-2025 MOA (Download PDF reader) - Section 622)
  • New Hire Incentive: New hires may be eligible for a one-time New Hire Incentive of $1,500, subject to the applicable section of the SEIU 721 MOA 2022-2025 (Download PDF reader), Article 38.
  • Educational Incentive - An educational incentive of 2.5% for completion of an associate's degree, 3.5% for completion of a bachelor's degree, OR 5% for completion of a graduate's degree.
  • Bilingual Incentive - Proficiency levels by exam are $.69 per hour (Level I), $1.00 per hour (Level II), or $1.32 per hour (Level III).
  • Vacation Accrual – New regular, full-time employees shall accrue approximately 14 days of vacation a year for the first 10,400 hours or 5 years of service; vacation accruals increase at 5, 11, 12, 13, 14, 15 and 20 years of service topping out at 26 days a year and 400 hours of vacation hours banked. 
  • Vacation Leave Redemption – After 14,560 hours of continuous County service an employee may elect to "cash in" or redeem up to 80 hours of vacation accrued in the same year after using 80 hours of vacation in the preceding 12 months. 
  • Sick Leave - Full time regular employees accrue 3.08 hours of sick leave per pay period with an advance at hire of 40.04 hours which will be balanced at the completion of 13 bi-weekly pay periods. 
  • Deferred Compensation - Eligible to participate in the County's 401(k) Shared Savings Plan and/or the Section 457 Plan. This position is eligible for up to a 3.00% match on your 401(k) contributions. 
  • Health Plans – You are afforded an annual flexible credit allowance for purchasing medical, dental, and/or vision insurance from a group of authorized plans.
  • Flexible Spending Accounts - Pre-tax benefit towards eligible medical, dental and vision care expenses.
  • Pension Plan - Participation in the County's defined pension plan, which has reciprocity with CalPERS.
  • Holidays - 12 paid days per year which, includes a scheduled floating holiday.

THE POSITION:
Under the general supervision of a Program Administrator, the incumbents will support Ambulatory Care licensed and registered professional staff in providing community outreach, education, case coordination and assistance.
 
The incumbents will provide field-based services and outreach to patients who are seeking medical services. The current vacancies are throughout the County of Ventura.
 
IDEAL CANDIDATES:
The ideal candidates will possess excellent customer and public service skills. They will be able to manage time effectively and will be skilled in planning and organization. They will be familiar with documentation standards and will have previous case-management experience. Finally, they will be willing and able to work independently as well as on a team.
 
AGENCY/DEPARTMENT: Health Care Agency - Ambulatory Care
Community Health Worker is represented by the Service Employees' International Union (SEIU) and is eligible for overtime compensation.
 
The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for these positions only. There are currently eight (8) vacancies within Health Care Agency - Ambulatory Care.TENTATIVE SCHEDULE
OPENING DATE: October 17, 2023
CLOSING DATE: Continuous until business needs have been met.
FIRST REVIEW OF APPLICATIONS: Beginning the week of October 23, 2023 and on an ongoing, frequent basis thereafter.Duties may include but are not limited to the following:
  • Assists in implementing Care Plan; coordinates client follow-up of immunizations, specialty or diagnostic referrals, specimen collection and skin testing; interprets and monitors patient/client compliance; provides and reinforces health education and Care Plan; helps patients/clients make appointments and provides assistance for them to keep appointments; reports observations and findings to nurse or clinician;
  • Patient/client intake; assists in the initial patient/client assessments and follow up including basic intake interviews and screenings; assists individuals or families to define problems and needs, and place them in order of priority;
  • Keeps appropriate records; documents client encounters and progress notes consistent with agency and program requirements; documents findings of individual encounters and focus groups and community interactions;
  • Provides health education; uses standardized or approved curricula, provides basic information verbally, written and with visual aids; teaches basic health, living skills and home safety; instructs patients/clients on how to collect specimens to be examined in the laboratory; educates patients/clients on communicable diseases and conducts follow up investigations;
  • Addresses barriers to care by linking patients/clients to community resources and provides care management services;
  • Assists patients/clients with enrolling in programs that support preventative health and treatment;
  • Follows program guidelines and regulations, including eligibility and billing.
  • Meets productivity and quality goals set by agency;
  • Health navigation: provides information, training, referrals, and/or support to assist beneficiaries to: access health care, understand the health care system, and/or engage in their own care; connect to community resources necessary to promote a beneficiary’s health; address health care barriers, including connecting to medical translation/interpretation and/or transportation services; and/or address health-related social needs.
  • Participates in care team huddles, meetings, and required training;
  • Coordinates resources and outreach to encourage and facilitate the use of appropriate preventive services; and
  • Performs related duties as required.
These are entrance requirements to the exam process and assure neither continuance in
the process nor placement on an eligible list.

EDUCATION, TRAINING, and EXPERIENCE:
High school diploma, GED or equivalent, PLUS any combination of education and experience which has led to the acquisition of the required knowledge and abilities. 

NECESSARY SPECIAL REQUIREMENTS:

  • Must possess or obtain a valid California driver license by date of hire.
  • Experience and ability to work in a Windows environment and using Microsoft Word required.
DESIRED:
  • Case management experience with at-risk population.
  • College course work in social sciences, ethnic studies, health sciences, communications, or related fields.
  • Bilingual (English/Spanish)
  • Customer service experience or experience interacting with the public.
  • Knowledge of community services organizations that provide services to Ventura County at-risk populations.
  • Obtain a Community Health Worker Certificate from a California Department of Health Care Access and Information approved training program.
  • Previous experience working with relevant Federal (i.e. Social Security) and/or State (i.e., Medi-Cal) Programs or private health insurance companies.
KNOWLEDGE, SKILLS AND ABILITIES:
 
Some to thorough knowledge of: community service organizations that provide services to a diverse, at-risk population; medical terminology; community agency program policies and procedures; specialized health related topics such as communicable diseases and preventative health maintenance; medical guidelines; group and family dynamics.
 
Skilled in: Personal computer usage , including proficiency with Microsoft Word and Excel; providing customer service in a case management setting. 
 
Ability to: Maintain case management notes in electronic health record; monitor client compliance with health care plan; effectively communicate with individuals and groups from diverse cultural, at-risk, socio-economic backgrounds; develop trusting and effective working relationships with a diverse client population; work effectively in a team environment.FINAL FILING DATE: This is a continuous recruitment and may close at any time; therefore apply as soon as possible if you are interested in it. TRAINING & EXPERIENCE EVALUATION : A Training and Experience Evaluation (T&E) is a structured evaluation of the job application materials submitted by a candidate, including the written responses to the supplemental questionnaire. The T&E is NOT a determination of whether the candidate meets the stated requirements; rather, the T&E is one method for determining who are the better qualified among those who have shown that they meet the stated requirements. In a T&E, applications are either scored or rank ordered according to criteria that most closely meet the business needs of the department. Candidates are typically scored/ranked in relation to one another; consequently, when the pool of candidates is exceptionally strong, many qualified candidates may receive a score or rank which is moderate or even low resulting in them not being advanced in the process.

ORAL EXAMINATION - 100%: A job-related Oral Examination will be conducted to evaluate and compare participating candidates' knowledge, skills, and abilities in relation to those factors which job analysis has determined to be essential for successful performance of the job. Candidates must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list.
 If there are three (3) or fewer qualified applicants, an examination will not be conducted. Instead, a score of seventy percent (70%) will be assigned to each application, and each applicant will be placed on the eligible list.

Applicants successfully completing the exam process may be placed on an eligible list for a period of one (1) year.
