Summary:

Our award-winning client is seeking a Director Case Management, RN to join their team.We are seeking a dynamic and results-oriented Facility Case Management Director to lead our growing department.

Responsibilities:

● Provide strategic direction for the Case Management department, overseeing daily operations, staffing, budget, and performance improvement initiatives. Foster a collaborative and high-performing team environment.

● Ensure the department adheres to best practices in case management, care coordination, and discharge planning. Work closely with physicians, nurses, social workers, and other stakeholders to ensure continuity of care and optimal resource utilization.

● Analyze departmental data to identify areas for improvement and implement strategies to enhance quality, efficiency, and patient outcomes.

● Ensure department adherence to all organizational policies, regulations, and standards.

● Develop and maintain strong relationships with key internal and external stakeholders, including the CMO, CNO, department leaders, and external agencies.

● Lead the department through change initiatives, fostering a culture of continuous improvement and innovation.

Required Qualifications:

● Bachelor's degree required, Master's in Nursing, Health Administration, or Business Administration preferred.

● Registered Nurse (RN) license or Licensed Clinical Social Worker (LCSW) license required.

● Minimum 3 years of experience in acute care hospital case management, with 2 years in a leadership role preferred.

● Proven ability to lead, motivate, and develop a high-performing team.

● Excellent communication, interpersonal, and collaboration skills.

● Strong analytical and problem-solving skills.

● Ability to work independently and manage multiple priorities.

● Demonstrated commitment to patient-centered care and quality improvement.

Job Type: Full-time

Pay: $140,000.00 - $150,000.00 per year
