Job Details
Position Summary:
Responsible for the ongoing psychosocial assessment and development/ implementation of treatment/discharge plans for patients receiving both inpatient and outpatient care and services. The social worker collaborates with the multidisciplinary team, lending insight and expertise regarding the psychosocial and post-services needs of patients, offering recommendations for appropriate discharge planning and general disposition following treatment. Acts as a liaison between hospital and family when patient is transferred to other facilities.
Minimum Requirements:
Education/Licenses/Certifications:
Masters degree in Social Work Required
Apply for a Counselor Agency Affiliate Registration, State of Washington, within 30 days of hire
Compensation Range:
E18 - Min: $29.53-Max: $42.49
