Position Summary:
The Hospital Social Worker is responsible for cross continuum care coordination, psychosocial assessments, and psychosocial interventions. This individual works with providers, peers, community colleagues, and others to orchestrate care across the health care continuum, to identify opportunities to continually improve patient care and services, to improve population health, and to achieve collaborative practices that exemplify MultiCare Health Systems commitment to patient centered care and community engagement.
Minimum Requirements:
3 years Hospital or equivalent experience
Health industry knowledge preferred
Healthcare financial assistance knowledge preferred
Leadership experience a plus
Education/Licenses/Certifications:
Masters Degree in Social Work
Licensed in the state of Washington
Licensed Independent Clinical Social Worker (LICSW); OR Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain their LICSW within 4 years
Compensation Range:
E20 - Min: $33.81-Max: $48.65
