Valley Comprehensive Community Mental Health... is Hiring a Medical Care Coordinator Near Morgantown, WV

The Medical Care Coordinator is responsible for serving Valley’s consumers who only receive doctor’s services. The Medical Care Coordinator’s duties include assessment of consumer’s medical needs, completing and ensuring consumer treatment strategies stay current, and advocating on behalf of the consumer; linking/referring consumers to needed community resources; Tier 1 updates, Treatment Request Forms (TRF’s), medication refill request forms, case closures, and referring consumers to Service Coordination when more services are needed.

The Medical Care Coordinator interfaces Support Staff, Quality Assurance, Outpatient Nursing and all other Medical Staff to keep consumers in compliance with treatment strategies. The ideal candidate for this position is highly organized, proficient in medical charting and has clear professional communication skills.


ESSENTIAL DUTIES of Service Coordination:

  • Reviews current clinical documentation to gather information to complete the Tier 1 Update.
  • Keep Treatment Request Forms (TRF’s) current to ensure services are authorized.
  • Completes Medication Refill Request forms and forwards them to nursing staff.
  • Completes Case Closures as identified by Supervisor
  • Refers consumers to Service Coordination when more services are needed.
  • Uses clear, friendly, communication skills with consumers and other professionals.
  • When High-End Services are identified as a need, presents the case to Supervisor to be assigned to a High-End Service Coordinator.
  • Provide education, linkage, and referral to community resources appropriate to the level of consumer need.
  • Coordinate Transfer/Discharge meetings and follow-up with other service providers on the provision of treatment to allow for continuity of services.
  • Access the necessary resources detailed in a consumer’s treatment plan, make referrals to qualified service providers and resources.
  • Monitor to ensure that the consumer’s health and safety needs are addressed.
  • Further develop and monitor the progress of the consumer’s treatment strategies.
  • Staff participates in group or individual supervision with supervisor.
  • Returns telephone calls and requests for information in a timely manner (Within 72 hours).
  • Assess consumer’s needs and evaluate progress toward treatment goals.
  • Use electronic conferencing technology, such as Zoom, Microsoft Teams, and Conference Call features on a Telephone to facilitate consumer interaction when face-to-face is not possible.
  • Keep treatment strategies and reviews current for the assigned caseload.
  • Completes and submits Electronic Medical Record documentation as required by Valley, OHFLAC, BMS, CMS, and other licensing agencies (Notes Including Direct Support Progress Notes, Master Treatment Plans, etc.) completed accurately and timely and will accurately and appropriately reflect services the consumer(s) has received.
  • Reviews consumer’s Electronic Medical Record (EMR) charts and documentation from other programs or service providers on a monthly basis to ensure coordination of services.
  • Provide crisis services and intervention and/or assistance with consideration of the principles of consumer choice and least restrictive alternatives.
  • Meets direct consumer service time (DCST) standards set forth.

MINIMUM QUALIFICATIONS:

  • Ability to perform Essential Duties as outlined below.
  • BA/BS in Human Service Field.
  • Six months to one-year experience with MI and/or SUD population.
  • Ability to comply with all of Valley’s Policies and procedures.
  • Ability to comply with Consumer’s Rights.
  • Ability to comply with Valley’s and Division departmental safety procedures.
  • Ability to read, write and speak the English language.
  • Valid Driver’s License
  • Ability to obtain and maintain CPR/First Aid, and MANDT Certifications.
  • Organization Skills and Ability to adhere to strict guidelines
