The Medical Care Coordinator is responsible for serving Valley’s consumers who only receive doctor’s services. The Medical Care Coordinator’s duties include assessment of consumer’s medical needs, completing and ensuring consumer treatment strategies stay current, and advocating on behalf of the consumer; linking/referring consumers to needed community resources; Tier 1 updates, Treatment Request Forms (TRF’s), medication refill request forms, case closures, and referring consumers to Service Coordination when more services are needed.
The Medical Care Coordinator interfaces Support Staff, Quality Assurance, Outpatient Nursing and all other Medical Staff to keep consumers in compliance with treatment strategies. The ideal candidate for this position is highly organized, proficient in medical charting and has clear professional communication skills.
