$5,000.00 relocation bonus and $10,000 sign-on bonus with a 2-year commitment for qualified candidates.The UVA Children's Hospital Division of Pediatric Hematology/Oncology seeks an RN Care Coordinator. The successful candidate will join a dedicated and caring multidisciplinary team consisting of MDs, NPs, a licensed clinical social worker, a psychologist, clinical research coordinators, an education consultant, and other professional staff to enable the best possible service to these complex patients and their families. The role involves management and coordination across the entire care continuum in partnership with our providers and support staff. This will include ensuring proper scheduling of outpatient visits, hospital admissions, procedures, imaging studies, and other elements of patient care, as well as securing prior authorizations for treatment and coordinating care with home health providers. Participation in pre-clinic care coordination conferences will be required daily to ensure a proactive approach to patient care needs. Bidirectional communication with patients and families in person, by telephone, and via the electronic medical record is a critical component of this position.

Position Compensation Range: $65,249.60 - $107,660.80 AnnuallyMINIMUM REQUIREMENTS- Education: Graduate of accredited nursing program. Bachelor of Science in Nursing required within 5 years of hire.
- Experience: 1 year of relevant experience.
- Licensure: Licensed to practice as a registered nurse in the Commonwealth of Virginia. American Heart Association (AHA) Healthcare Provider BLS certification required.
