TheUWMC Digestive Health CenterhasTWOoutstanding opportunities forFull-Time (40 hours a week, Day Shift) Patient Care Coordinators.The Patient Care Coordinator (PCC) acts as primary liaison for UWMC/UW Medicine and represents departmental physician(s) with the hospital, patient and family, referring physicians, outside agencies, staff from outside hospitals and general public by providing information and advice regarding UWMC hospital services and the Digestive Health Center.
PRIMARY RESPONSIBILITIES
Acts as primary liaison for UWMC/UW MedicineResponsible for reviewing registration data such as demographic, insurance, referral, next of kin, etc.Acts as a liaison between patients, consulting physician, referring physician or outside agency to coordinate patient appointments, admission, or surgery.Responsible for reviewing registration data with patient for scheduled inpatient surgery/admission or outpatient surgery/procedures. E.g. demographic, insurance, referral, next of kin etc., and updating online or providing information to appropriate personnel so that an update may be done.Coordination of pre-admission tests and procedures.Coordinates the direct transfer of patients from outside hospitals to UWMC with physicians, medical staff and admitting through the Patient Flow supervisor.Acts as liaison between patient, physicians, OR, nurses, other clinics, nursing units, admitting, Financial Counseling, blood bank, lab, Radiology, OT, PT, Prosthetics, etc. for the purpose of coordinating all pre-op planning and preparation.Schedules and communicates pre-op/post-op planning and preparation to patient. Sends appointment and other information to patients.Responsible for requesting necessary surgical equipment, tools, supplies, devices, and implants (will supply necessary info for custom implants).Responsible for scheduling, rescheduling, and canceling surgeries on ESI scheduling system.Follow-up care, including assistance with discharge from clinic or hospital, arranging transportation, and attending hospital discharge rounds.REQUIRED QUALIFICATIONS
A Bachelor's Degree in a medically related or public relations field AND two years of experience in acomplexpatient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians, and proficiency in medical terminology OR equivalent education/experience.
